Sleep Dysfunction of Post Traumatic Stress Disorder: Lessons from Southeast Asia
Original Air Date: Wednesday, February 18, 2009 at 1 PM EST

But the tigers come at night,
With their voices soft as thunder,
As they tear your hope apart,
As they turn your dream to shame

From I Dreamed a Dream, LES MISERABLES

Post Traumatic Stress Disorder (PTSD) is the most frequently reported psychiatric consequence of traumatic events and of man-made disasters.  This presentation will focus on the dysfunctional sleep symptomatology which occurs in PTSD with special reference given to the atrocities of the Vietnam War and the genocide which occurred at the hands of the Khmer Rouge in the “Killing Fields” of Cambodia.

Anxiety and sleep dysfunction are frequently interlaced.  Post Traumatic Stress Disorder (PTSD) is a well defined diagnostic entity whose main feature is that of anxiety, as outlined in the Diagnostic and Statistical Manual of Mental Disorders (DSM-IV).  Some individuals develop PTSD upon exposure to life-threatening stress, and as a result, may experience recurrent distressing recollections and re-experiencing of the trauma, persistent avoidance of stimuli associated with the trauma, and persistent symptoms of increased arousal.  In addition, PTSD-related sleep dysfunction may include recurrent nightmares or distressing dreams of the event, as well as difficulty falling or staying asleep.

During a recent trip to Cambodia and Vietnam as ambassadors for the American Psychiatric Association, a group of psychiatrists from across the nation gained new perspectives and incite all these decades later regarding the magnitude of the aftermath of Southeast Asian atrocities inclusive of the development of PTSD symptomatology within their communities, and the need for improved psychiatric treatments.  Aside from some conventional, as well as newer unconventional medications, non-medication interventions such as behavioral-cognitive therapy, improved sleep hygiene, nightmare imagery rehearsal training, and others have shown promise in treating the dysfunctional sleep in patients with PTSD here in the United States.
